Most red eyes get diagnosed somewhere between the bathroom mirror and the search bar. The verdict is nearly always the same. Pink eye. It has become the default label for anything that turns the white of the eye an unhappy shade of red, partly because it is genuinely common and partly because it is the only name most people know.
The trouble is that redness is one of the least specific signals the human body produces. A blood vessel dilates and the eye looks red. That is all redness tells you. It cannot tell you whether the problem sits on the surface, inside the eye, or in the pressure system that keeps the whole thing inflated. A harmless burst capillary and a sight-threatening emergency can both present as “my eye is red,” and the difference between them is usually found in what accompanies the redness rather than in the redness itself.
This is not an argument for panic. The overwhelming majority of red eyes are benign and resolve on their own. It is an argument for knowing which handful of features separate the boring cases from the ones that need attention today.
Redness Is A Sign, Not A Diagnosis
The white of your eye is the sclera, a tough fibrous shell. Over it lies the episclera, a thin vascular layer, and over that sits the conjunctiva, a transparent membrane running from the edge of the cornea to the inside of your eyelids. Each of these layers has its own blood supply, and each can become inflamed independently.
That anatomy explains why the pattern of redness carries information. Redness spread diffusely across the whole white, heaviest toward the eyelids and fading as it approaches the colored part of the eye, points to the conjunctiva. Redness concentrated in a ring right where the cornea meets the white, sometimes described as a ciliary flush, points inward, toward the cornea or the interior of the eye. A single sharply bordered patch of solid red that looks painted on is a different phenomenon entirely. A wedge of redness confined to one sector suggests inflammation of a specific layer.
You do not need to make these calls yourself. But noticing them, and being able to describe them to a clinician, changes the quality of the answer you get.

Before naming any condition, run through these. They are roughly the same questions an ophthalmologist asks in the first ninety seconds.
Does it hurt, or does it just bother you? There is a real difference between gritty, scratchy, itchy, or “something in my eye,” and genuine pain. Surface irritation is uncomfortable. Deep aching, throbbing, or pain that wakes you at night belongs to a different category.
Has your vision changed? Not blurred by a film of discharge that clears when you blink, but actually changed. Reduced acuity, haze that persists, halos around lights, or new floaters are all significant.
Does light hurt? Mild squinting in bright sun is normal. Photophobia that makes you want to shut the curtains, or pain in the affected eye when light is shone into the other eye, is a sign of inflammation inside the eye rather than on its surface.
What is coming out of it? Watery, stringy, or thick and colored. Each points somewhere different.
One eye or both, and how fast? Bilateral onset over a day or two suggests something systemic or allergic. Sudden, severe, one-sided onset shortens the list considerably.
Often it is. Conjunctivitis comes in three main flavors, and telling them apart matters mostly because the treatments differ so much.
Viral is the most common. It usually starts in one eye and moves to the other within a few days, produces watery discharge, and frequently arrives alongside a cold or sore throat. There is often a tender lymph node just in front of the ear. Adenovirus is the usual culprit, and it is stubbornly contagious, sometimes for ten days or more. It is also self-limiting. There is no antiviral for it, and there does not need to be.
Bacterial tends to produce thicker, yellow or green discharge, and the classic complaint of lids gummed shut on waking. In otherwise healthy adults it often clears without treatment, though antibiotic drops can shorten it modestly.
Allergic is bilateral, dominated by itching rather than pain, and usually accompanied by a runny nose or a known trigger. Itching is the single most useful discriminator here. Infective conjunctivitis is irritating; allergic conjunctivitis makes you want to rub your eyes raw.

Antibiotic Reflex, And Why It Usually Misses
Here is something that reframes the whole conversation. A large analysis of a United States managed care network found that roughly 58% of people newly diagnosed with acute conjunctivitis filled a prescription for topical antibiotics, and about one in five of those prescriptions was a combined antibiotic and corticosteroid drop.[1] Yet up to 80% of conjunctivitis is viral, where antibiotics do nothing at all.[2]
The steroid combination is the more concerning half of that statistic. Topical corticosteroids applied to an undiagnosed red eye can be genuinely dangerous. If the underlying problem is a herpes simplex corneal infection, steroids can accelerate it. If it is a fungal keratitis, the same. Steroids can also raise intraocular pressure in susceptible people. This is why a leftover bottle from a previous episode is not a safe shortcut, no matter how similar this episode feels.
Ones That Look Alarming And Usually Are Not
Subconjunctival hemorrhage. A tiny vessel under the conjunctiva breaks and blood spreads into a flat, vivid, sharply outlined red patch. It looks like something out of a horror film and it is, in almost every case, the ocular equivalent of a bruise. There is no pain, no discharge, and no change in vision. It typically clears over one to two weeks, fading through yellow-green on the way out. Coughing, sneezing, straining, vomiting, or rubbing can all set it off, and blood thinners make it more likely. The American Academy of Ophthalmology’s explanation of subconjunctival hemorrhage is reassuring reading if you have just noticed one.
Two caveats. Recurrent episodes deserve a blood pressure check and, occasionally, clotting tests. And a hemorrhage that follows blunt trauma, particularly one that wraps all the way around the eye, is not automatically benign and needs to be examined.
Episcleritis. A wedge or sector of the white becomes bright red, often over a few hours. It feels mildly uncomfortable rather than painful, vision stays normal, and light does not bother you. It settles on its own, sometimes with lubricating drops, and tends to recur. It is unnerving because of how localized and abrupt it looks, but it is not a threat to sight.
Ones That Look Ordinary And Are Not
Iritis, also called anterior uveitis. The redness may be modest, concentrated around the edge of the cornea. What sets it apart is the combination of a deep ache, real photophobia, and sometimes blurred vision or floaters. A useful bedside clue: shine a light into the unaffected eye and the affected one hurts. Uveitis is often linked to autoimmune conditions, and it recurs. Untreated, it can cause the iris to stick to the lens and drive up eye pressure. The Academy’s overview of uveitis explains why prompt treatment matters more than the outward appearance suggests.
Scleritis. Much rarer, much more serious. The pain is the defining feature: deep, boring, radiating to the brow or jaw, bad enough to wake people at night. The redness has a bluish or violet undertone and the eye is tender to touch through the lid. Scleritis is frequently the first visible sign of a systemic inflammatory disease such as rheumatoid arthritis or vasculitis, which means the eye exam is only half the workup.
Keratitis and corneal injury. Anything that breaches or infects the cornea produces a painful, light-sensitive, watery red eye, usually with the sensation that something is stuck under the lid. Corneal abrasions from a fingernail or a stray branch, foreign bodies, and infectious ulcers all live here. Herpes simplex is a recurring cause and a commonly missed one, since it can masquerade as ordinary conjunctivitis; the NHS page on herpes simplex eye infections describes how it presents and why it needs specific treatment.
Acute angle-closure glaucoma. This is the true emergency. Drainage inside the eye is suddenly blocked and pressure climbs fast. The eye becomes intensely red and painful, vision blurs, halos appear around lights, and the pupil sits mid-sized and sluggish. Many people also feel nauseated and vomit, which is why some cases get sent home from emergency departments as gastroenteritis or migraine. The eye may feel firm to the touch compared with the other side. Sight can be lost within hours, so this is a same-day, drop-everything problem. Shingles around the eye. Herpes zoster affecting the ophthalmic nerve produces a painful red eye alongside a blistering rash on one side of the forehead or scalp. A rash on the tip of the nose is a particularly meaningful warning sign, because the same nerve branch supplies structures inside the eye. Antiviral treatment works best when started within roughly 72 hours of the rash appearing, so this is not a wait-and-see situation.
Cellulitis around the eye. Swelling, redness, and warmth of the eyelids with fever, especially in children, needs assessment. If moving the eye hurts, the eye bulges, or vision is affected, the infection may have spread behind the orbital septum, which is a hospital-level problem.
Slow Burn: Dry Eye And Blepharitis
Not every red eye is an event. A large share of chronic, low-grade redness comes from an unstable tear film or inflamed eyelid margins, and these are the cases most often misfiled as “pink eye that keeps coming back.”
Dry eye is paradoxical enough to fool people: it can cause reflex watering, so the eye is simultaneously dry and streaming. It tends to worsen through the day, in air conditioning, on screens, and on flights. Blepharitis adds crusting at the lash line, morning stickiness, and recurrent styes. Warm compresses, deliberate lid hygiene, and preservative-free lubricants do more here than any antibiotic ever will, though results take weeks rather than days. If You Wear Contact Lenses, The Rules Change
This deserves its own paragraph because the risk profile is genuinely different. Contact lens wear raises the risk of microbial keratitis, an infection of the cornea that can scar, perforate, or require a corneal transplant in severe cases. The CDC has estimated that keratitis and contact lens complications account for around 930,000 outpatient visits and 58,000 emergency department visits in the United States each year.[3]
The practical rule is simple. A contact lens wearer with a red, painful eye should take the lenses out, keep them out, wear glasses, and be examined promptly. Not tomorrow if it is still bad. Promptly. Symptoms that would be unremarkable in a non-wearer carry a different weight here, and the CDC’s explanation of contact lens related eye infections is worth reading before you need it rather than after.
Sleeping in lenses, topping off old solution rather than replacing it, and any contact between lenses and water are the three behaviors most consistently tied to serious infection.
Red Eyes In Children
Two situations override normal reassurance. Any red or discharging eye in a baby under 28 days old needs same-day medical assessment, because neonatal conjunctivitis can involve organisms that damage the cornea quickly. And significant eyelid swelling with fever, or a child who will not open the eye, warrants examination rather than watchful waiting.
Beyond that, most childhood conjunctivitis is viral and self-limiting. School exclusion policies vary widely and often exceed what the medical evidence supports, so it is worth checking the specific policy rather than assuming a week at home is required.

For nearly every red eye, the advice is to get assessed before doing anything. Chemical exposure is the exception. If a cleaning product, solvent, or any unknown chemical gets into the eye, start flushing immediately with clean running water or saline and keep going for at least 15 to 20 minutes, holding the lids open. Then seek care, and bring the container or a photo of the label. Alkaline substances such as oven cleaner, bleach, and cement penetrate faster and cause more damage than acids, and minutes matter more than anything a clinician can do afterward.
What Not To Reach For
Leftover antibiotic or steroid drops. Covered above. The steroid risk in particular is real.
Redness-relief drops. Vasoconstrictors whiten the eye by squeezing the vessels shut. With repeated use, they produce rebound redness that is worse than the original, and they mask a symptom you may need to track.
An eye patch. Patching does not speed healing of corneal abrasions and is specifically unsafe in contact lens wearers.
Numbing drops beyond a single clinical dose. Topical anesthetics are toxic to the cornea with repeated use and can cause serious damage. They are never a take-home remedy.
Tweezers, cotton buds, or fingers aimed at anything embedded in the eye.
Contact lenses, until an eye care professional says otherwise.
What Actually Helps While You Wait
For a red eye with no warning features, the useful measures are unglamorous. Cool compresses for comfort and swelling. Preservative-free artificial tears as often as you like. Warm compresses and lid cleaning if the eyelid margins are involved. Over-the-counter antihistamine or mast cell stabilizer drops if itching dominates and allergy is the likely cause. Separate towels, frequent handwashing, and no shared pillows if it appears infectious. Glasses instead of lenses.
When To Stop Waiting
Seek care the same day if you have any of the following:
Moderate or severe eye pain, as distinct from irritation
Any reduction in vision, or halos around lights
Marked sensitivity to light
Nausea or vomiting accompanying a red, painful eye
A red eye in a contact lens wearer
Chemical exposure, or injury from anything traveling at speed
A rash on the forehead, scalp, or nose tip alongside the red eye
An irregularly shaped pupil
Redness in a newborn, or in anyone whose immune system is suppressed
Redness following recent eye surgery or an intraocular injection
Symptoms that are worsening rather than plateauing after two to three days
What Happens At The Eye Exam
People sometimes delay because they imagine something invasive. It is not. A standard red eye assessment involves reading a letter chart, a look with a bright light, and an examination at a slit lamp, which is a microscope you rest your chin against. A drop of fluorescein dye, which glows under blue light, reveals scratches and ulcers on the cornea within seconds. Eye pressure is measured painlessly after a numbing drop. If needed, the pupil is dilated for a view of the back of the eye. The whole thing usually takes twenty minutes, and the reassurance of an actual answer tends to be worth considerably more than another hour of searching.
Short Version
Redness alone is not informative. Pain, vision change, and light sensitivity are. If your red eye is itchy, gritty, watery, or gummy, and you can see normally and it does not really hurt, you are almost certainly in benign territory and time is on your side. If it aches deeply, blurs your vision, makes light unbearable, or belongs to someone wearing contact lenses, that is a different conversation, and it should be had today rather than next week.
The eye is unusually good at signaling trouble. The signals just are not in the color.
